Referral Services:

HormoneMD provides referral and connection services. HormoneMD connects you with and facilitates your connection to licensed healthcare providers who provide healthcare services. HormoneMD itself does not provide healthcare services. By using HormoneMD, you acknowledge that all telemedicine services, including consultations, prescriptions, and healthcare advice, are provided by these licensed healthcare providers and not by HormoneMD.

Case Management Services:

HormoneMD provides case management services, specifically coordination and advocacy services to reduce healthcare costs through better care coordination. We work with licensed healthcare providers and other parties to arrange services and advocate on your behalf to ensure you receive proper care and support. Licensed healthcare providers may use HormoneMD-branded communications to facilitate your healthcare services. HormoneMD does not store or retain your Protected Health Information (PHI) or other sensitive data. See our Privacy Policy for data handling details.

Accurate Information:

You are responsible for providing accurate and complete information when interacting with HormoneMD. Providing false or incomplete information may result in an inability to connect you to a licensed healthcare provider.

Lawful Use:

You agree to use HormoneMD solely for lawful purposes. You will not engage in any activity that disrupts or interferes with the functionality of our website or its associated systems.

HormoneMD provides its website on an “as is” and “as available” basis without warranties of any kind, express or implied. We do not guarantee the accuracy, completeness, or reliability of any information on our website. HormoneMD does not warrant that the website will be error-free or uninterrupted.
